India Health Collaborative
Aligning with WHO and Government of India frameworks, our collaborative aims to implement a comprehensive healthcare model emphasising universal health coverage, addressing social determinants of health, and strengthening health systems. This model includes community participation to ensure culturally relevant interventions and preventive health education to enhance awareness of non-communicable diseases and health issues like WASH and menstrual health. Accessible diagnostics will be provided through community centes and mobile units for early detection and monitoring. Primary, secondary, and tertiary care will be delivered via local healthcare providers, strengthening referral protocols to ensure smooth patient transitions. Continuum of care will be supported through tele-health, extending services to remote areas, and robust referral mechanisms. A digital health platform will track patient journeys and manage population health data, informing public health decisions. Continuous training and capacity building for the health workforce will adapt to evolving challenges, and a knowledge management system will capture and share insights, driving health practice improvements. This comprehensive approach will ensure equity, integration, and evidence-based practices in healthcare delivery.
